Online Portrait Request Form

The American Fallen Soldiers Project provides, upon request, hand painted original portraits of America's fallen heroes to their families free of charge.

We also have available reproduction prints on canvas for other immediate family members who did not receive the original painting.  *Limit one full-size canvas reproduction at no cost per family.

Each portrait and canvas reproduction print is mounted and framed and of the highest quality.

Portraits may be requested by the primary next of kin or other family member of a fallen American military serviceman who has died during active duty in the Afghanistan, Iraq, or previous wars.  However, if the portrait is requested by a distant or non-family member, we do ask that the original portrait be given to the immediate family and a canvas reproduction print will be made available to the additional family members.

INSTRUCTIONS -- HOW TO REQUEST A PORTRAIT OF YOUR FALLEN SERVICEMAN --

Online requests and photograph submission is preferred, but if you cannot submit your form and pictures online, you may download and print the Mail-In Request form below and mail your submission along with your photographs to the address listed.  Photographs submitted via mail will not be returned.

STEP 1:    Click on the   Online Request Form  link to begin the submission process.  If you are unable to complete your Request Form online, we do have a downlable PDF form (click on the Mail-In Request Form Link at the bottom of this page).

STEP 2:    While submitting the  Online Request Form , please attach a minimum of 3 high-resolution, quality photographs of your loved one, however you can send additional pictures if you desire.  Please choose pictures that reveal the detail of their face clearly and up close, when possible.  These type of pictures are necessary to help the artist capture your loved one's personality and physical characteristics.  If you need an example of the type of pictures you should choose, please see the  Fallen Soldiers Gallery  on our website.

The images submitted do not have to be in military dress, but the actual portrait will be chosen from one of the submitted photographs at the discretion of the artist.  We do not blend multiple photographs or remove goggles, hats, etc. for the portrait as this may distort the final results, so please submit photographs that you would want to be representative of your loved one for a lifetime.

STEP 3:
Upon completing your  Online Request Form   you will receive a confirmation email that we have received your information.

If you are using the  Mail-In Request Form  you may send it to --

STEP 4:    Our desire, and the heart of The American Fallen Soldiers Project, is to make a profound and lasting impact while also providing a place where you can spend time with your loved one when you need them most.  We work diligently to restore a sense of life and presence of our fallen American heroes on canvas and it is this commitment to excellence that defines our portraits, our staff, and our organization.  Because of the amount of time and care we put into each portrait and because we try to personally deliver as many as possible in a special memorial event or privately, our portrait completion time can take up to 24-28 months.

STEP 5:    When it is time to begin your portrait we will contact you either via email or telephone to discuss your portrait’s delivery date, time and type of delivery.  Then, during the painting process, Phil Taylor will talk with you, usually via email, regarding any questions he may have about your loved one while also updating you as to the portrait’s progress.

If you have any questions or need further information regarding the portrait submission process or our organization, please contact us at  info@americanfallensoldiers.com  or (214) 387-7769.  And, please feel free to contact us after submitting your Request at any time.  We are here for you.

Thank you for allowing Phil Taylor and The American Fallen Soldiers Project the honor to paint your fallen loved one.  It is our mission to honor, respect and forever memorialize those who have sacrificed their lives protecting our freedom while serving in our Armed Forces.
